Geriatric Emergency Medicine (GEM)
The Geriatric Emergency Medicine Program, also called GEM, provides specialized support for older adults who come to the Emergency Department.
At West Haldimand General Hospital, the GEM program helps seniors return home safely and maintain independence whenever possible.
What Is GEM?
GEM is a specialized Emergency Department service for adults aged 65 and older.
It focuses on identifying health and safety concerns that may affect an older adult’s ability to manage at home after an emergency visit.
The goal is to support safe discharge and connect patients to the right care in the community.
Who GEM helps
GEM may be involved if you or your loved one has:
- A recent change in health or memory
- Trouble walking, balance issues, or recent falls
- Difficulty with daily activities at home
- Safety or well-being concerns
- Frequent Emergency Department visits
How to access GEM
You do not need to request this service.
Emergency Department staff may refer you to the GEM nurse if they feel it would be helpful.
To receive a GEM assessment, you must:
- Be 65 years or older
Younger patients may be considered at the discretion of the care team - Live in your own home
This includes assisted living, supportive housing, or a retirement home - Be referred by Emergency Department staff
- Provide consent to take part
After-hours referrals receive follow-up by phone.
What to expect during a GEM assessment
The GEM nurse will complete a detailed assessment, with your consent.
With your permission, family members or caregivers may be involved.
The assessment focuses on areas that affect safety and independence, including:
- Memory and thinking
- Mood and mental health
- Mobility and fall risk
- Physical function
- Pain
- Medications
- Nutrition and hydration
- Skin health
- Sleep
- Continence
- Social supports
Other team members, such as a physiotherapist or occupational therapist, may assist if needed.
What happens after
Before you leave the Emergency Department, the GEM nurse helps create a plan for next steps.
This plan may include:
- A summary shared with your family doctor or care provider
- Referrals to Home and Community Care Services
- Connections to community senior support programs

The GEM program supports safe discharge, reduces the risk of health decline, and helps older adults stay independent at home.
